Drug Class: Dihydropyridine Calcium Channel Blocker | Form: Oral Tablet | Prescription Status: Prescription Only (Rx) — DRAP Registered

What is Amodip 10mg Tablet? Amodip 10mg contains Amlodipine Besylate — the maximum approved dose of this third-generation dihydropyridine calcium channel blocker with an exceptionally long 35 to 50-hour half-life providing smooth and sustained blood pressure control from a single daily dose. The 10mg strength is prescribed for patients whose hypertension or angina remains inadequately controlled on the standard 5mg dose — delivering maximum calcium channel blockade for superior blood pressure reduction and antianginal effect. Amlodipine's gradual onset avoids the reflex tachycardia seen with shorter-acting calcium channel blockers and its long half-life means occasional missed doses have minimal impact on blood pressure control. It is one of the most extensively studied antihypertensives globally with a proven record of cardiovascular protection in high-risk patients — available in convenient 30-tablet blister packs for month-long treatment courses.

What is Amodip 10mg Used For?

  • Hypertension — maximum dose for blood pressure inadequately controlled on 5mg (ہائی بلڈ پریشر جو کم خوراک سے کنٹرول نہ ہو)
  • Chronic stable angina — prevention of exertional chest pain from coronary artery disease (سینے کے درد کی روک تھام)
  • Vasospastic angina — Prinzmetal angina caused by coronary artery spasm (خون کی نالیوں کی اینٹھن سے سینے کا درد)
  • Cardiovascular risk reduction — reducing heart attack and stroke risk in high-risk hypertensive patients (دل کے دورے اور فالج کی روک تھام)
  • Hypertension with asthma or COPD — preferred safe alternative to beta-blockers in respiratory disease
  • Hypertension with diabetes — Amlodipine does not impair glucose metabolism or mask hypoglycaemia

How Does it Work? Amlodipine selectively blocks L-type voltage-gated calcium channels in vascular smooth muscle — preventing calcium entry that triggers arterial smooth muscle contraction. At the maximum 10mg dose this calcium channel blockade is most complete — producing the strongest peripheral and coronary arterial dilation achievable from Amlodipine therapy. Reduced vascular resistance lowers blood pressure while coronary vasodilation simultaneously improves myocardial oxygen delivery — addressing both the blood pressure elevation and the coronary supply-demand imbalance responsible for anginal chest pain. Amlodipine's gradual slow calcium channel binding kinetics avoid abrupt vasodilation and reflex sympathetic activation — providing smooth sustained cardiovascular effects without compensatory tachycardia throughout the full 24-hour dosing period.

Dosage and Administration

⚠️ Never stop suddenly — gradual dose reduction under cardiologist supervision required. Takes 7 to 14 days for full blood pressure effect at 10mg. Do not exceed 10mg daily — this is the maximum approved dose. Ankle swelling is significantly more common at 10mg than 5mg — report persistent worsening to doctor. Can be taken with or without food at the same time each day.

Indication Usual Adult Dose Frequency Notes
Hypertension — maximum dose 10mg Once daily When 5mg provides insufficient control
Chronic stable angina 5 – 10mg Once daily Titrated to symptom and heart rate response
Vasospastic angina 5 – 10mg Once daily As prescribed by cardiologist
Elderly patients 5mg Once daily Use 10mg with extreme caution — increased hypotension and fall risk
Hepatic impairment 2.5 – 5mg Once daily 10mg only under strict specialist supervision

Who Should NOT Take Amodip 10mg?

  • Allergy to Amlodipine, any dihydropyridine calcium channel blocker, or Amlodipine Besylate
  • Severe hypotension — systolic blood pressure below 90mmHg
  • Cardiogenic shock
  • Unstable angina — acute coronary syndrome excluding vasospastic angina
  • Significant aortic stenosis — severe narrowing of aortic valve; 10mg contraindicated
  • Acute myocardial infarction — within first month; use with extreme caution
  • Severe liver impairment — 10mg contraindicated; start at 2.5mg under supervision
  • Pregnancy — particularly first trimester; consult cardiologist
  • Breastfeeding — Amlodipine passes into breast milk
  • Children under 6 years for hypertension indication

Side Effects

Common: Significant ankle swelling — peripheral oedema markedly more common at maximum 10mg dose (ٹخنوں کی سوجن — 10mg پر زیادہ عام ہے — عموماً بے ضرر), facial flushing and warmth (گرمی کا احساس), mild to moderate headache, dizziness especially on standing, mild fatigue, mild palpitations — ankle swelling at 10mg is the most frequent reason for dose reduction back to 5mg.

Serious — Stop and seek emergency help immediately: Severe hypotension with fainting — particularly significant at maximum dose in volume-depleted patients (بے ہوشی — فوری مدد لیں), paradoxical worsening of angina shortly after starting maximum dose — rare vasodilatory reflex (سینے میں شدید درد — فوری ہسپتال جائیں), severe allergic reaction — sudden widespread rash, swelling of face or throat, difficulty breathing (فوری ہسپتال جائیں), significant liver toxicity — yellowing of skin or eyes (یرقان — فوری مدد لیں), erythromelalgia — severe burning pain and redness of extremities — rare but distressing, severe skin reactions including blistering or peeling.

Drug Interactions

Medicine Interaction
Simvastatin Amlodipine 10mg significantly increases Simvastatin exposure — limit Simvastatin strictly to maximum 20mg daily; serious myopathy and rhabdomyolysis risk at higher statin doses
Cyclosporine / Tacrolimus Amlodipine significantly increases their blood levels at 10mg — increased nephrotoxicity and toxicity risk; monitor closely
Ketoconazole / Itraconazole Significantly increase Amlodipine blood levels — excessive hypotension risk at maximum 10mg dose
Rifampicin Dramatically reduces Amlodipine blood levels — significantly reduced antihypertensive and antianginal effectiveness
Other antihypertensives Additive blood pressure lowering — increased hypotension risk particularly significant at maximum 10mg dose
Sildenafil (Viagra) Additive vasodilation — significant hypotension risk amplified at 10mg; take Sildenafil minimum 4 hours after Amodip
Lithium Amlodipine may rarely increase Lithium neurotoxicity — monitor
Grapefruit juice Inhibits Amlodipine metabolism — significantly increases blood levels and hypotension risk at 10mg; avoid completely
Tacrolimus Amlodipine increases Tacrolimus blood levels — monitor levels and kidney function
Alcohol (شراب) Increases vasodilation and hypotension risk — amplified at maximum 10mg dose; avoid excessive consumption

Storage: Store below 25°C in a cool, dry place. Protect from moisture and direct sunlight. Keep in original blister pack — 30 tablets per pack. Keep out of reach of children (بچوں کی پہنچ سے دور رکھیں). Do not use after expiry date.

FAQs

Q: Why is ankle swelling so much more noticeable at Amodip 10mg compared to the 5mg dose? Ankle swelling is a dose-dependent effect of Amlodipine — at the maximum 10mg dose peripheral arterial dilation is most complete, increasing capillary hydrostatic pressure in the legs and driving more fluid into surrounding tissue than at lower doses. While generally harmless and not a sign of heart or kidney problems, persistent significant ankle swelling at 10mg may require stepping down to 5mg and adding a second antihypertensive medicine from a different class to maintain blood pressure control without the oedema burden.

Q: Is 10mg the highest Amlodipine dose — can it ever be exceeded for very difficult to control blood pressure? 10mg is the absolute maximum approved daily dose of Amlodipine — exceeding this provides no additional blood pressure or antianginal benefit while significantly amplifying ankle swelling, hypotension, and other vasodilatory side effects. For blood pressure remaining uncontrolled at 10mg Amlodipine the clinically correct approach is adding a second antihypertensive from a completely different class — such as an ARB, ACE inhibitor, or thiazide diuretic — rather than attempting to exceed the maximum dose.

Q: Can Amodip 10mg be taken safely alongside Atorvastatin for patients who also need cholesterol treatment? Yes — Amlodipine has no clinically significant interaction with Atorvastatin at standard doses. However the significant interaction is specifically with Simvastatin — Amlodipine 10mg increases Simvastatin exposure dramatically, increasing myopathy risk at Simvastatin doses above 20mg. Patients requiring statin therapy alongside Amodip 10mg should use Atorvastatin or Rosuvastatin rather than Simvastatin — or limit Simvastatin to a maximum of 20mg daily.
