What is Otosporin Ear Drops 5ml?

Otosporin Ear Drops 5ml is a triple-combination ear drop medicine containing three active ingredients — Hydrocortisone, a corticosteroid (steroid), and two antibiotics, Neomycin and Polymyxin B. It is used to treat bacterial infections of the outer ear canal (کان کی نالی کا بیکٹیریل انفیکشن) — commonly known as otitis externa or swimmer's ear — where infection and inflammation are both present together. The two antibiotics work as a team to kill a broader range of bacteria than either could cover alone, while Hydrocortisone rapidly reduces the swelling, itching, redness, and pain caused by the body's inflammatory response. This three-in-one combination makes Otosporin particularly effective for outer ear infections where broad antibacterial coverage and inflammation control are both needed at the same time. Otosporin Ear Drops must only be used as prescribed by your doctor and must never be used if the eardrum is perforated or damaged.

What is Otosporin Ear Drops Used For?

Otosporin Ear Drops 5ml is approved for the treatment of the following conditions:

  • Otitis Externa (Outer Ear Canal Infection) — bacterial infection of the skin lining the ear canal causing pain, swelling, discharge, redness, and itching (کان کی بیرونی نالی کا بیکٹیریل انفیکشن)
  • Swimmer's Ear — infection and inflammation of the outer ear canal caused by water remaining in the ear after swimming, bathing, or prolonged exposure to humid or damp conditions
  • Infected Eczema of the Ear Canal — bacterial infection developing on top of eczematous or inflamed skin inside the ear canal requiring both antibiotic and anti-inflammatory treatment
  • Acute and Chronic Otitis Externa — both sudden-onset and long-standing outer ear canal infections with significant inflammation, redness, and itching
  • Inflammatory Outer Ear Conditions with Secondary Bacterial Infection — ear canal skin conditions where the body's inflammatory reaction has been complicated by bacterial overgrowth
  • Post-Traumatic Ear Canal Infections — bacterial infection and inflammation following minor scratching, trauma, or skin breakdown in the ear canal

How Does Otosporin Ear Drops Work?

Otosporin Ear Drops combine three active ingredients that work through different but highly complementary mechanisms to treat infection, kill bacteria, and control inflammation simultaneously:

Hydrocortisone (Corticosteroid — Steroid): When bacteria infect the ear canal, the body's immune system triggers an inflammatory response — releasing chemicals called prostaglandins, histamines, and cytokines that cause redness, swelling, heat, itching, and pain in the delicate skin of the ear canal. While this is a natural protective reaction, excessive inflammation makes symptoms significantly worse, narrows the ear canal, and hinders recovery. Hydrocortisone is a mild-to-moderate potency corticosteroid that suppresses this inflammatory response by blocking the production of these inflammatory chemicals — rapidly reducing swelling, redness, itching, and pain in the ear canal. By reducing canal swelling, Hydrocortisone also helps the two antibiotic components penetrate deeper into the canal to reach the bacteria more effectively. Being a milder steroid than Betamethasone or Dexamethasone, Hydrocortisone is well suited for ear use — providing meaningful anti-inflammatory relief with a lower risk of steroid-related side effects.

Neomycin (Aminoglycoside Antibiotic): Neomycin kills bacteria by entering bacterial cells and binding irreversibly to their ribosomes — the structures bacteria use to manufacture the proteins essential for their survival, growth, and reproduction. By disrupting this protein production process, Neomycin causes bacteria to malfunction and die. It is effective primarily against Gram-positive bacteria and many Gram-negative organisms commonly found in outer ear infections including Staphylococcus aureus and Proteus species.

Polymyxin B (Antibiotic): Polymyxin B kills bacteria through a completely different mechanism — it attacks the outer cell membrane of Gram-negative bacteria, disrupting its structural integrity and causing the bacteria to leak their internal contents and die. It is particularly effective against Gram-negative organisms such as Pseudomonas aeruginosa — one of the most common and difficult-to-treat bacteria in outer ear infections. Together, Neomycin and Polymyxin B cover a significantly broader spectrum of bacteria than either antibiotic alone — reducing the chance of treatment failure and providing more comprehensive bacterial clearance.

Dosage and Administration

⚠️ For use in the ear only — never swallow ear drops or place in the eyes. Never use these drops if the eardrum is perforated. Always follow your doctor's prescribed dose and duration exactly. Do not use for longer than prescribed — prolonged use of steroid and antibiotic ear drops carries important risks.

Patient Group Dose Frequency
Adults and children over 12 years 3 drops into the affected ear 3 to 4 times daily
Children under 12 years 1 to 2 drops into the affected ear 3 times daily — as prescribed by doctor

How to Apply Otosporin Ear Drops Correctly:

  1. Wash your hands thoroughly before handling the bottle
  2. Warm the bottle gently by holding it in your hands for 1 to 2 minutes — applying cold drops directly into the ear can cause sudden dizziness
  3. Shake the bottle gently before use if instructed on the label
  4. Tilt your head to one side so the affected ear faces directly upward
  5. Gently pull the outer ear upward and backward (for adults) or downward and backward (for young children) to fully straighten and open the ear canal
  6. Hold the dropper just above the ear opening — do not insert the dropper tip inside the ear canal as this can contaminate the solution and cause injury
  7. Apply the correct number of drops carefully
  8. Gently press the small flap of skin (tragus) over the ear opening several times to help the drops travel deep into the ear canal and coat the canal walls
  9. Keep your head tilted for 1 to 2 minutes to allow the drops to settle and work properly
  10. Use a clean tissue to gently catch any excess drops that overflow — do not push cotton wool or anything else inside the ear canal
  11. Repeat for the other ear if both ears are affected and your doctor has prescribed treatment for both
  12. Replace the cap tightly and store the bottle correctly after every use
  • Complete the full prescribed course even if symptoms improve — stopping early allows bacteria to survive and risks the infection returning or worsening
  • Treatment is typically continued for 7 days — do not extend use beyond 10 days without doctor review
  • If symptoms do not begin to improve within 3 to 4 days of starting treatment, contact your doctor for reassessment

Who Should NOT Use Otosporin Ear Drops?

Do not use these drops if you or your child:

  • Have a perforated eardrum (پھٹا ہوا پردہ کان) — the most critical contraindication. Neomycin passing through a perforation into the middle and inner ear can cause permanent and irreversible hearing loss and balance damage
  • Have a suspected middle ear infection (otitis media — درمیانی کان کا انفیکشن) — middle ear infections are a different condition requiring different treatment — usually oral antibiotics — assessed by a doctor
  • Have a viral ear infection — including infections caused by Herpes simplex or Varicella. The steroid Hydrocortisone suppresses the immune response and can worsen viral infections significantly
  • Have a fungal ear infection (otomycosis — پھپھوندی سے کان کا انفیکشن) — the steroid component can worsen fungal infections — antifungal treatment is required instead
  • Are allergic to Hydrocortisone, Neomycin, Polymyxin B, any other aminoglycoside antibiotic (e.g., Gentamicin, Tobramycin), or any corticosteroid medicine
  • Have grommets (ear tubes) fitted — Neomycin passing through grommets into the middle ear can cause inner ear damage — consult your doctor before use
  • Have had ear surgery — particularly involving the eardrum or middle ear structures — consult your surgeon before using any antibiotic ear drops
  • Are under 1 year of age — consult a specialist paediatrician before use in very young infants

Tell your doctor before use if:

  • You are unsure whether your eardrum is intact — your doctor must examine the ear and confirm the eardrum is completely intact before prescribing these drops
  • There is any discharge from the ear that may indicate a ruptured or perforated eardrum
  • You have a history of recurrent or chronic ear infections
  • You have previously had an allergic or sensitisation reaction to any ear drops — particularly Neomycin-containing preparations, as Neomycin is a well-known and common cause of contact allergy
  • You have any existing hearing loss or balance problems
  • You are pregnant or breastfeeding — your doctor will carefully assess safety before prescribing

Side Effects

Like all medicines, Otosporin Ear Drops can cause side effects. Because these drops are applied locally in the ear canal, side effects are generally mild and limited to the ear area. Not everyone will experience them.

Common Side Effects (relatively frequent):

  • Mild temporary stinging or burning sensation immediately after applying the drops — usually settles within a few minutes
  • Mild itching inside the ear canal — often temporary and improves as infection and inflammation clear with treatment
  • Feeling of fullness or pressure in the ear — usually resolves as swelling reduces during the treatment course
  • Temporary dizziness immediately after application — most commonly caused by applying cold drops — always warm the bottle in your hands first to prevent this

Serious Side Effects (less common — seek medical help immediately):

  • Allergic contact dermatitis to Neomycin — Neomycin is one of the most frequently reported causes of topical antibiotic skin allergy. Signs include worsening redness, intense itching, new rash, swelling, weeping discharge, or blistering in or around the ear — stop the drops immediately and see your doctor. This reaction can be mistaken for a worsening infection so always report any deterioration in symptoms to your doctor promptly
  • Severe allergic reaction — sudden widespread rash, swelling of the face, lips, or throat, difficulty breathing — seek emergency help immediately
  • Fungal superinfection (otomycosis) — prolonged use of steroid-antibiotic ear drops suppresses local immunity and can allow fungi to overgrow in the ear canal. Signs include new or worsening itching, thick unusual discharge, or an unusual smell from the ear developing after initial improvement — inform your doctor immediately
  • Hearing loss or changes in hearing — any new or worsening hearing loss during treatment must be reported to your doctor immediately — stop the drops if any hearing change is noticed
  • Balance problems or persistent dizziness — new dizziness or balance disturbance during treatment must be reported promptly — Neomycin can affect the inner ear's balance organ if it reaches it through a perforated eardrum
  • Worsening of symptoms — if pain, discharge, swelling, or redness increases after 3 to 4 days of treatment rather than improving, stop the drops and see your doctor — the infection may require reassessment
  • Signs of systemic steroid absorption — very rare with correctly used short-term topical ear drops but report any unusual weight gain, facial puffiness, or mood changes to your doctor if drops are used beyond the recommended duration

If you notice any serious side effects, stop using the drops immediately and go to the nearest hospital or doctor.

Drug Interactions

Otosporin Ear Drops are applied directly into the ear canal and very little medicine is absorbed into the bloodstream with correct short-term use. Significant systemic drug interactions are therefore unlikely. However the following are important:

Consideration Details
Other ear drops Do not use other ear drops at the same time without specific doctor advice — allow at least 30 minutes between different ear preparations to avoid diluting the medicine
Systemic aminoglycoside antibiotics (e.g., Gentamicin, Tobramycin, or Amikacin injections) If the patient is also receiving aminoglycoside antibiotics by injection, inform your doctor immediately — combined aminoglycoside exposure requires careful monitoring for hearing and kidney effects
Loop diuretics (e.g., Furosemide) Can significantly enhance the hearing toxicity of aminoglycosides — inform your doctor if the patient is taking Furosemide alongside Neomycin-containing ear drops
Systemic corticosteroids (e.g., Prednisolone tablets or Dexamethasone injections) If the patient is already receiving oral or injected steroids, inform your doctor — combined steroid exposure from multiple sources should be monitored
Immunosuppressant medicines Hydrocortisone may further reduce local immune response — inform your doctor if taking any immunosuppressant medicines
Known allergy to any aminoglycoside antibiotic Do not use Neomycin or Polymyxin B-containing drops if the patient has a known allergy to any aminoglycoside — cross-sensitivity is possible

Always inform your doctor about all medicines, ear preparations, vitamins, or supplements currently being used.

Storage Instructions

  • Store below 25°C in a cool, dry place
  • Keep away from direct sunlight and moisture
  • Do not refrigerate or freeze the ear drops
  • Keep out of reach of children (بچوں کی پہنچ سے دور رکھیں)
  • Do not use after the expiry date printed on the bottle or outer box
  • Keep the bottle tightly closed after each use to prevent contamination of the solution
  • Discard the bottle 4 weeks after first opening — do not use beyond this period even if liquid remains in the bottle
  • Do not use if the solution appears cloudy, discoloured, or contains any visible particles

Frequently Asked Questions (FAQs)

What is Otosporin Ear Drops used for?

Otosporin Ear Drops are used to treat bacterial infections of the outer ear canal — otitis externa and swimmer's ear — where infection and inflammation are present together. The two antibiotics, Neomycin and Polymyxin B, work as a team to kill a wide range of bacteria responsible for the infection, while Hydrocortisone rapidly reduces the swelling, redness, itching, and pain caused by the body's inflammatory response.

Why does Otosporin contain two antibiotics instead of one?

Neomycin and Polymyxin B kill bacteria through completely different mechanisms and are effective against different types of bacteria. Neomycin is stronger against Gram-positive bacteria and some Gram-negative organisms, while Polymyxin B specifically targets difficult-to-treat Gram-negative bacteria — particularly Pseudomonas aeruginosa, which is one of the most common causes of outer ear infections. Using both antibiotics together in the same drop provides much broader bacterial coverage than either could achieve alone — significantly reducing the chance of treatment failure due to resistant or mixed bacterial infections.

How is Otosporin different from Betnesol-N Ear Drops?

Both Otosporin and Betnesol-N are combination antibiotic-steroid ear drops but they differ in their components. Otosporin uses Hydrocortisone — a milder steroid — combined with two antibiotics (Neomycin and Polymyxin B) for broader bacterial coverage. Betnesol-N uses Betamethasone — a more potent steroid — combined with Neomycin only. Otosporin may be preferred when broader antibiotic coverage is needed or a milder steroid is more appropriate. Your doctor will prescribe the most suitable option based on your specific ear condition and the bacteria likely to be responsible.

Why is it so important not to use Otosporin with a perforated eardrum?

The eardrum is a protective barrier between the outer ear canal and the delicate middle and inner ear structures. If the eardrum has a hole or tear, Neomycin — an aminoglycoside antibiotic — can pass through into the inner ear where it is directly toxic to the hair cells responsible for hearing and the nerve endings responsible for balance. This damage is permanent and cannot be reversed. Your doctor must always examine your ear and confirm the eardrum is completely intact before prescribing any aminoglycoside-containing ear drops including Otosporin.

I previously had a reaction to Neomycin cream on my skin — can I use Otosporin?

If you have previously had an allergic or sensitisation reaction to Neomycin — whether in a skin cream, ointment, or eye or ear drop — you must inform your doctor before using Otosporin. Neomycin is a well-known and common cause of allergic contact dermatitis and the allergy persists once sensitisation has occurred. Using Neomycin again after sensitisation can cause a significant worsening reaction in the ear. Your doctor will assess your previous reaction and prescribe a Neomycin-free alternative if necessary.

My symptoms are getting worse after 3 days of using Otosporin — what should I do?

If your ear pain, discharge, swelling, or itching is getting worse rather than better after 3 to 4 days of treatment, stop the drops and see your doctor as soon as possible. Worsening symptoms can indicate several possibilities — the infection may be caused by bacteria not sensitive to Neomycin or Polymyxin B, you may have developed an allergic contact reaction to Neomycin, the infection may be fungal rather than bacterial, or the condition affecting your ear may not be a simple outer ear infection and needs further investigation. Do not continue using the drops if your symptoms are deteriorating.
