Drug Class: Angiotensin II Receptor Blocker (ARB) | Form: Oral Tablet | Prescription Status: Prescription Only (Rx) — DRAP Registered

What is Misar 80mg Tablet? Misar 80mg contains Telmisartan — a long-acting ARB with the longest half-life among all ARBs at approximately 24 hours, providing smooth and consistent blood pressure control throughout the entire day and night from a single daily dose. Unlike ACE inhibitors, Telmisartan does not cause the persistent dry cough that leads many patients to discontinue treatment — making it better tolerated for long-term hypertension management. Beyond blood pressure lowering, Telmisartan provides significant cardiovascular and renal protection — reducing the risk of heart attack, stroke, and kidney damage in high-risk patients. Misar is also available as combination formulations including Misar AM with Amlodipine and Misar H with Hydrochlorothiazide for patients requiring additional blood pressure lowering agents.

What is Misar 80mg Used For?

  • Hypertension — high blood pressure management (ہائی بلڈ پریشر)
  • Cardiovascular risk reduction — preventing heart attack and stroke in high-risk patients (دل کے دورے اور فالج کی روک تھام)
  • Diabetic nephropathy — kidney protection in type 2 diabetic patients with microalbuminuria (ذیابیطس میں گردوں کی حفاظت)
  • Chronic kidney disease — slowing progression in patients with proteinuria
  • Post-myocardial infarction — cardiovascular protection after heart attack under cardiologist supervision
  • Heart failure — as alternative to ACE inhibitors in intolerant patients

How Does it Work? Telmisartan selectively blocks AT1 angiotensin II receptors — the receptors through which angiotensin II exerts its powerful vasoconstricting, aldosterone-stimulating, and sodium-retaining effects. By blocking these receptors, Telmisartan prevents angiotensin II from causing blood vessel constriction and fluid retention — producing sustained vasodilation and blood pressure reduction. Additionally Telmisartan has partial PPAR-gamma agonist activity — providing additional metabolic benefits including improved insulin sensitivity and lipid profile beyond simple blood pressure lowering. Its exceptionally long 24-hour half-life ensures no end-of-dose blood pressure rebound — providing stable protection even if a dose is taken slightly later than usual.

Dosage and Administration

⚠️ Never stop suddenly — abrupt discontinuation causes dangerous rebound hypertension. Takes 4 to 8 weeks for full blood pressure lowering effect. Monitor kidney function and blood potassium regularly. Can be taken with or without food at the same time each day. Volume depletion must be corrected before starting therapy.

Indication Usual Adult Dose Frequency Notes
Hypertension 40 – 80mg Once daily Same time each day
Cardiovascular risk reduction 80mg Once daily As prescribed by cardiologist
Diabetic nephropathy 40 – 80mg Once daily Regular kidney monitoring required
Heart failure — ACE inhibitor intolerant 40 – 80mg Once daily Under specialist supervision

Who Should NOT Take Misar 80mg?

  • Allergy to Telmisartan or any ARB medicine
  • Pregnancy — Telmisartan is strictly contraindicated in all trimesters; causes serious foetal kidney damage and death
  • Breastfeeding
  • Severe liver impairment or biliary obstructive disorders — Telmisartan is primarily excreted through bile
  • Severe kidney impairment or bilateral renal artery stenosis
  • Concurrent use of Aliskiren in patients with diabetes or kidney impairment
  • Hereditary angioedema or prior angioedema with any ARB or ACE inhibitor
  • Severe hypotension — systolic blood pressure below 90mmHg
  • Significant volume depletion — dehydration or sodium depletion

Side Effects

Common: Mild dizziness especially on standing (اٹھتے وقت چکر), mild headache, mild fatigue, mild upper respiratory symptoms — generally very well tolerated with significantly lower side effect burden than ACE inhibitors including no dry cough.

Serious — Stop and seek emergency help immediately: Angioedema — sudden swelling of face, lips, tongue or throat with difficulty breathing (چہرے اور گلے کی سوجن — فوری ہسپتال جائیں), severely low blood pressure with fainting particularly after first dose or in dehydrated patients (بے ہوشی — فوری مدد لیں), significantly raised blood potassium — hyperkalaemia causing muscle weakness and irregular heartbeat (پٹھوں کی کمزوری — فوری ڈاکٹر کو بتائیں), significant worsening of kidney function — reduced urination with ankle swelling and weight gain, severe allergic reaction with widespread rash.

Drug Interactions

Medicine Interaction
ACE inhibitors (Enalapril, Lisinopril) Increased hyperkalaemia and acute kidney failure risk — avoid dual RAAS blockade
Aliskiren Increased hyperkalaemia and kidney damage — contraindicated in diabetes and kidney disease
Potassium supplements / Potassium-sparing diuretics Significantly increased hyperkalaemia risk — monitor potassium closely
NSAIDs (Ibuprofen, Diclofenac) Reduce antihypertensive effectiveness and worsen kidney function
Digoxin Telmisartan increases Digoxin peak blood levels — monitor Digoxin levels closely
Lithium Telmisartan increases Lithium blood levels — toxicity risk; monitor closely
Other antihypertensives / diuretics Additive blood pressure lowering — increased hypotension risk
Warfarin Telmisartan may slightly reduce Warfarin effect — INR monitoring recommended
Alcohol (شراب) Increases blood pressure lowering and dizziness risk — avoid excessive consumption

Storage: Store below 25°C in a cool, dry place. Protect from moisture and direct sunlight. Keep in original packaging. Keep out of reach of children (بچوں کی پہنچ سے دور رکھیں). Do not use after expiry date.

FAQs

Q: Why is Telmisartan in Misar preferred over ACE inhibitors for many patients? Telmisartan provides equivalent blood pressure lowering and cardiovascular protection to ACE inhibitors without causing the persistent dry cough that affects up to 20% of ACE inhibitor users — making long-term treatment significantly more tolerable and improving treatment compliance for hypertension management.

Q: Can I stop Misar 80mg once my blood pressure readings become consistently normal? Never stop suddenly — blood pressure returns to dangerously high levels quickly after stopping and abrupt discontinuation causes rebound hypertension. Hypertension is a lifelong condition requiring consistent management and any dose changes must always be made under cardiologist supervision.

Q: Is Misar 80mg safe to take during pregnancy? No — Telmisartan is strictly contraindicated throughout all three trimesters of pregnancy. It causes serious foetal kidney damage, reduced amniotic fluid, and potentially fatal foetal harm. Women of childbearing age must use effective contraception and inform their doctor immediately if pregnancy occurs while taking Misar.
