What is Macrobac 500mg Capsule? Macrobac 500mg contains Azithromycin Dihydrate — a broad-spectrum macrolide antibiotic at the full standard therapeutic dose used for most adult bacterial infections. Azithromycin's unique pharmacokinetic profile — achieving tissue concentrations several times higher than blood concentrations and maintaining therapeutic levels for days after the last dose — allows a 3 to 5 day course to provide bacterial eradication equivalent to 7 to 10 day courses of other antibiotics. The 500mg daily dose is the standard adult treatment dose providing optimal tissue concentrations for respiratory, skin, throat, and sexually transmitted infections in a single once-daily capsule. Its convenient short course significantly improves treatment compliance and completion — a critical factor in achieving cure and preventing antibiotic resistance development. Like all antibiotics, Macrobac must only be used for confirmed bacterial infections under medical supervision.

What is Macrobac 500mg Capsule Used For?

  • Community-acquired pneumonia — bacterial lung infection requiring full-dose treatment (نمونیا)
  • Acute exacerbation of chronic bronchitis — bacterial lower respiratory tract infection (برونکائٹس)
  • Acute sinusitis — bacterial sinus infection with facial pain and congestion (ناک کی ہڈی کا انفیکشن)
  • Tonsillitis and pharyngitis — bacterial throat and tonsil infection (گلے کا انفیکشن)
  • Acute otitis media — bacterial middle ear infection (کان کا انفیکشن)
  • Skin and soft tissue infections — cellulitis and moderate bacterial skin infections (جلد کا انفیکشن)
  • Chlamydia and non-gonococcal urethritis — sexually transmitted bacterial infections (جنسی امراض)
  • Typhoid fever — Salmonella typhi infection under medical supervision (ٹائیفائیڈ بخار)

How Does it Work? Azithromycin binds to the 50S ribosomal subunit of bacterial cells — blocking protein synthesis essential for bacterial growth, reproduction, and survival. This bacteriostatic action halts bacterial multiplication, allowing the immune system to eliminate the infection. At 500mg Azithromycin achieves tissue concentrations 10 to 100 times higher than plasma concentrations in infected tissues — including lungs, tonsils, sinuses, skin, and genitourinary tract — providing superior antibiotic coverage at infection sites compared to many other antibiotic classes. Its tissue half-life of approximately 68 hours means therapeutic concentrations persist for several days after the final dose — extending antibacterial activity beyond the visible treatment course and preventing early relapse.

Dosage and Administration

⚠️ Always complete the full prescribed course even if symptoms resolve within 1 to 2 days — stopping early causes treatment failure and promotes antibiotic resistance. Take on an empty stomach or with a light meal — food reduces absorption of capsule formulation. Never self-prescribe antibiotics. Minimum 2 hours between Macrobac and antacids containing aluminium or magnesium.

Indication Dose Duration Notes
Respiratory infections 500mg once daily 3 to 5 days Take at same time daily
Tonsillitis / Pharyngitis 500mg once daily 3 days For penicillin-allergic patients
Skin and soft tissue infections 500mg on day one then 250mg 5 days total Loading dose then step-down
Sinusitis 500mg once daily 3 days As prescribed by doctor
Ear infections 500mg once daily 3 days Under medical supervision
Chlamydia / STI 1000mg single dose One day only Single administration
Typhoid fever 500mg once daily 7 days Under specialist supervision

Who Should NOT Take Macrobac 500mg?

  • Allergy to Azithromycin, Erythromycin, or any macrolide antibiotic
  • History of cholestatic jaundice or liver dysfunction associated with prior Azithromycin use
  • Severe liver disease — Azithromycin primarily hepatically metabolised and excreted
  • Known QT interval prolongation or history of cardiac arrhythmia — Azithromycin prolongs QT
  • Currently taking other QT-prolonging medicines without cardiology review
  • Myasthenia gravis — Azithromycin can seriously worsen neuromuscular blockade
  • Severe kidney impairment — use with caution; no dose adjustment typically needed for mild to moderate impairment
  • Pregnancy and breastfeeding — consult doctor before use

Side Effects

Common: Nausea, mild stomach discomfort, mild diarrhoea, mild abdominal cramping, mild headache — gastrointestinal effects are the most common at 500mg and are generally mild and self-limiting, resolving quickly after completing the short course.

Serious — Stop and seek emergency help immediately: Severe allergic reaction — sudden widespread rash, swelling of face, lips, tongue or throat, difficulty breathing (فوری ہسپتال جائیں), severe watery or bloody diarrhoea with fever suggesting Clostridium difficile infection — can occur up to 2 months after completing course (فوری مدد لیں), cardiac arrhythmia — palpitations, irregular heartbeat, or dizziness suggesting QT prolongation (دل کی دھڑکن میں خلل — فوری ڈاکٹر کو بتائیں), severe liver toxicity — yellowing of skin or eyes with upper abdominal pain (یرقان — فوری مدد لیں), significant hearing loss or tinnitus — rare with short courses but more common with prolonged high-dose use, severe skin reactions including blistering or peeling — Stevens-Johnson syndrome.

Drug Interactions

Medicine Interaction
QT-prolonging medicines (Amiodarone, Haloperidol, certain antibiotics, antifungals) Significantly increased risk of dangerous cardiac QT prolongation and arrhythmia — avoid combination or use under strict cardiological supervision
Warfarin / blood thinners Azithromycin increases anticoagulant effect — close INR monitoring essential during and after course
Digoxin Azithromycin significantly increases Digoxin blood levels through gut flora alteration — serious toxicity risk; monitor Digoxin levels closely
Cyclosporine Azithromycin increases Cyclosporine blood levels — nephrotoxicity and toxicity risk; monitor closely
Antacids containing aluminium or magnesium Reduce Azithromycin peak absorption — take Macrobac at least 2 hours before or after antacids
Ergotamine / Dihydroergotamine Increased risk of ergot toxicity — avoid combination
Nelfinavir (HIV medicine) Significantly increases Azithromycin blood levels — monitor for increased side effects
Statins (Atorvastatin, Simvastatin) Azithromycin may increase statin blood levels — increased myopathy risk with short antibiotic course

Storage: Store below 25°C in a cool, dry place. Protect from moisture and direct sunlight. Keep out of reach of children (بچوں کی پہنچ سے دور رکھیں). Do not use after expiry date.

FAQs

Q: Why does Macrobac 500mg only need to be taken for 3 to 5 days when most antibiotics require 7 to 10 days? Azithromycin's unique pharmacokinetics — achieving and maintaining very high tissue concentrations for days after the last dose — means the drug continues fighting infection long after the course ends. Tissue concentrations remain above minimum inhibitory concentrations for most susceptible bacteria for 5 to 7 days after the final 500mg dose — providing equivalent total antibacterial exposure to much longer courses of other antibiotics with better compliance.

Q: Can I stop Macrobac 500mg after 2 days if I feel completely better? Never stop early — feeling better within 1 to 2 days is expected as Azithromycin rapidly accumulates in infected tissues, but this does not mean all bacteria are eliminated. Stopping prematurely leaves surviving bacteria that can develop antibiotic resistance and cause a harder-to-treat relapse requiring a second, longer antibiotic course. Always complete the exact duration prescribed by your doctor.

Q: Is Macrobac 500mg safe for patients with heart conditions? Azithromycin prolongs the cardiac QT interval — increasing the risk of dangerous arrhythmias in patients with pre-existing QT prolongation, electrolyte imbalances, or those taking other QT-prolonging medicines. Always inform your doctor of any heart conditions, arrhythmias, or cardiac medicines before starting Macrobac. For patients with significant cardiac risk your doctor may choose an alternative antibiotic from a different class.
