What is Betnesol-N Ear Drops 7.5ml?

Betnesol-N Ear Drops 7.5ml is a combination ear drop medicine containing two active ingredients — Betamethasone, a potent corticosteroid (steroid), and Neomycin, a broad-spectrum aminoglycoside antibiotic. It is used to treat bacterial infections of the outer ear canal (کان کی نالی کا بیکٹیریل انفیکشن) where both infection and inflammation are present together. Betamethasone rapidly reduces the redness, swelling, itching, and discomfort caused by inflammation in the ear canal, while Neomycin directly kills the bacteria responsible for the infection. This dual action addresses both the cause and the symptoms of outer ear infections simultaneously — providing faster and more complete relief than an antibiotic alone. Betnesol-N Ear Drops must only be used as prescribed by your doctor and must never be used if the eardrum is perforated or damaged.

What is Betnesol-N Ear Drops Used For?

Betnesol-N Ear Drops 7.5ml is approved for the treatment of the following conditions:

  • Otitis Externa (Outer Ear Canal Infection) — bacterial infection of the skin lining the ear canal with associated inflammation, pain, swelling, and discharge (کان کی بیرونی نالی کا بیکٹیریل انفیکشن)
  • Swimmer's Ear — infection and inflammation of the outer ear canal caused by water remaining in the ear after swimming, bathing, or prolonged exposure to humid conditions
  • Infected Eczema of the Ear Canal — bacterial infection occurring alongside eczema or inflamed skin inside the ear canal where both antibiotic and anti-inflammatory treatment are needed simultaneously
  • Inflammatory Otitis Externa with Secondary Bacterial Infection — conditions where ear canal inflammation has become complicated by bacterial infection
  • Acute and Chronic Otitis Externa — both sudden-onset and long-standing outer ear infections with significant redness, swelling, and itching
  • Allergic Ear Canal Inflammation with Bacterial Superinfection — allergic skin reactions in the ear canal that have developed a secondary bacterial infection

How Does Betnesol-N Ear Drops Work?

Betnesol-N Ear Drops combine two medicines working through different but highly complementary mechanisms to treat both infection and inflammation together:

Betamethasone (Corticosteroid — Steroid): When bacteria infect the ear canal, the body's immune system triggers an inflammatory response — releasing chemicals called prostaglandins, histamines, and cytokines that cause redness, swelling, heat, itching, and pain in the ear canal tissues. While this is a natural defence reaction, excessive inflammation significantly worsens symptoms, narrows the ear canal, and makes the environment more difficult for antibiotics to work in. Betamethasone is a highly potent corticosteroid that suppresses this inflammatory response by blocking the production of these inflammatory chemicals. It rapidly reduces swelling, redness, itching, and pain — opening up the ear canal and creating a much better environment for healing and for the antibiotic to work effectively. Betamethasone is one of the most potent topical steroids available, providing fast and powerful anti-inflammatory relief even in severely inflamed ear canals.

Neomycin (Aminoglycoside Antibiotic): Neomycin kills bacteria by entering bacterial cells and binding irreversibly to their ribosomes — the structures bacteria use to produce the essential proteins needed for survival, growth, and reproduction. By disrupting this protein production process, Neomycin causes the bacteria to malfunction and die. It is effective against a wide range of bacteria commonly responsible for outer ear infections — including both Gram-positive and Gram-negative organisms. Neomycin acts locally within the ear canal with minimal absorption into the bloodstream — delivering a concentrated antibacterial effect precisely where it is needed.

Together, Betamethasone and Neomycin provide comprehensive treatment — quickly relieving the distressing symptoms of inflammation while simultaneously eliminating the bacterial infection causing them.

Dosage and Administration

⚠️ For use in the ear only — never swallow ear drops or place in the eyes. Never use these drops if the eardrum is perforated. Always follow your doctor's prescribed dose and duration exactly. Do not use for longer than prescribed — prolonged steroid use in the ear carries important risks.

Patient Group Dose Frequency
Adults and children over 12 years 2 to 3 drops into the affected ear Every 3 to 4 hours — or as prescribed by doctor
Children under 12 years 2 drops into the affected ear 3 times daily — as prescribed by doctor

How to Apply Betnesol-N Ear Drops Correctly:

  1. Wash your hands thoroughly before handling the bottle
  2. Warm the bottle gently by holding it in your hands for 1 to 2 minutes — applying cold drops directly into the ear canal can cause sudden dizziness
  3. Shake the bottle gently before use if instructed on the label
  4. Tilt your head to one side so the affected ear faces directly upward
  5. Gently pull the outer ear upward and backward (for adults) or downward and backward (for young children) to fully straighten and open the ear canal
  6. Hold the dropper just above the ear opening — do not insert the dropper tip inside the ear canal as this can contaminate the bottle and cause injury
  7. Apply the correct number of drops carefully
  8. Gently press the small flap of skin (tragus) over the ear opening several times to help the drops travel deep into the ear canal and coat it fully
  9. Keep your head tilted for 1 to 2 minutes to allow the drops to settle properly
  10. Use a clean tissue to gently catch any excess drops that overflow from the ear — do not push anything into the ear canal
  11. Repeat for the other ear if both ears are affected and your doctor has prescribed treatment for both
  12. Replace the cap tightly and store the bottle correctly after every use
  • Complete the full prescribed course even if symptoms improve before the course ends — stopping early allows bacteria to survive and risks the infection returning
  • Treatment is typically continued for 7 days — do not extend use beyond this without doctor review
  • If symptoms do not begin to improve within 3 to 4 days of starting treatment, contact your doctor for reassessment

Who Should NOT Use Betnesol-N Ear Drops?

Do not use these drops if you or your child:

  • Have a perforated eardrum (پھٹا ہوا پردہ کان) — the most critical contraindication. Neomycin passing through a perforation into the middle and inner ear can cause permanent, irreversible hearing loss and balance damage
  • Have a suspected middle ear infection (otitis media — درمیانی کان کا انفیکشن) — middle ear infections require different treatment and must be assessed by a doctor
  • Have a viral ear infection — including Herpes simplex or Varicella (chickenpox virus). Betamethasone steroid suppresses the immune response and can significantly worsen viral infections
  • Have a fungal ear infection (otomycosis — پھپھوندی سے کان کا انفیکشن) — the steroid Betamethasone can worsen fungal infections and antifungal treatment is required instead
  • Are allergic to Betamethasone, Neomycin, any other aminoglycoside antibiotic (e.g., Gentamicin, Tobramycin), or any corticosteroid medicine
  • Have grommets (ear tubes) fitted — Neomycin reaching the middle ear through grommets can cause inner ear damage — consult your doctor before use
  • Have had ear surgery — particularly involving the eardrum or middle ear — consult your surgeon before using any ear drops
  • Are under 1 year of age — consult a specialist paediatrician before use in very young infants

Tell your doctor before use if:

  • You are unsure whether your eardrum is intact — your doctor must examine the ear and confirm the eardrum is intact before prescribing steroid-antibiotic ear drops
  • There is any discharge from the ear that may indicate a ruptured eardrum
  • You have a history of recurrent ear infections or chronic ear disease
  • You have previously had an allergic or sensitisation reaction to any ear drops or aminoglycoside antibiotic medicines — Neomycin is a well-known cause of allergic contact dermatitis
  • You have any existing hearing loss or balance problems — aminoglycosides can worsen pre-existing hearing conditions if they reach the inner ear
  • You are pregnant or breastfeeding — your doctor will carefully assess safety before prescribing

Side Effects

Like all medicines, Betnesol-N Ear Drops can cause side effects. Because these drops are applied locally in the ear canal, side effects are generally mild and limited to the ear area. Not everyone will experience them.

Common Side Effects (relatively frequent):

  • Mild temporary stinging or burning sensation immediately after applying the drops — usually settles within a few minutes
  • Mild itching inside the ear canal — often temporary and improves as infection and inflammation clear
  • Feeling of fullness or pressure in the ear — usually resolves as swelling reduces with treatment
  • Temporary dizziness immediately after application — most commonly caused by applying cold drops — warming the bottle first prevents this

Serious Side Effects (less common — seek medical help immediately):

  • Allergic contact dermatitis to Neomycin — Neomycin is one of the most common causes of allergic skin sensitisation among topical antibiotics. Signs include worsening redness, intense burning itching, new rash, swelling, weeping, or blistering around or inside the ear — stop the drops immediately and consult your doctor. Importantly, this reaction can be confused with a worsening infection — always report any worsening symptoms to your doctor
  • Severe allergic reaction — sudden widespread rash, swelling of the face, lips, or throat, difficulty breathing — seek emergency help immediately
  • Fungal superinfection (otomycosis) — prolonged use of steroid-containing ear drops suppresses local immunity and can allow fungi to overgrow in the ear canal. Signs include new or worsening itching, thick or unusual discharge, or an unusual ear odour developing after initial improvement — inform your doctor
  • Hearing loss or changes in hearing — any new or worsening hearing loss during treatment must be reported to your doctor immediately — do not continue drops if hearing changes are noticed
  • Balance problems or persistent dizziness — new dizziness or balance disturbance during treatment must be reported promptly — aminoglycosides can affect the inner ear's balance organ if they reach it
  • Worsening of symptoms — if pain, discharge, swelling, or redness increases rather than improving after 3 to 4 days, stop and see your doctor — the infection may be caused by an organism not sensitive to Neomycin or may need reassessment
  • Signs of systemic steroid absorption — very rare with correctly used short-term topical ear drops but report any unusual weight gain, facial puffiness, or significant mood changes to your doctor if drops are used for extended periods

If you notice any serious side effects, stop using the drops immediately and go to the nearest hospital or doctor.

Drug Interactions

Betnesol-N Ear Drops are applied directly into the ear canal and very little medicine is absorbed into the bloodstream with correct short-term use. Significant systemic drug interactions are therefore unlikely. However the following are important considerations:

Consideration Details
Other ear drops Do not use other ear drops at the same time without specific doctor advice — allow at least 30 minutes between different ear preparations to avoid diluting either medicine
Systemic aminoglycoside antibiotics (e.g., Gentamicin, Tobramycin, or Amikacin injections) If the patient is also receiving aminoglycoside antibiotics by injection, inform your doctor immediately — combined aminoglycoside exposure requires careful monitoring for hearing and kidney effects
Loop diuretics (e.g., Furosemide) Can enhance the hearing toxicity of aminoglycosides — inform your doctor if the patient is taking Furosemide alongside Neomycin-containing ear drops
Systemic corticosteroids (e.g., Prednisolone tablets or injections) If the patient is already taking oral or injected steroids, inform your doctor — combined steroid exposure from multiple sources should be monitored
Immunosuppressant medicines Betamethasone may further reduce local immune response — inform your doctor if taking any immunosuppressant medicines
Known allergy to any aminoglycoside antibiotic Do not use Neomycin-containing drops if the patient has a known allergy or sensitisation to any aminoglycoside antibiotic

Always inform your doctor about all medicines, ear preparations, vitamins, or supplements currently being used.

Storage Instructions

  • Store below 25°C in a cool, dry place
  • Keep away from direct sunlight and moisture
  • Do not refrigerate or freeze the ear drops
  • Keep out of reach of children (بچوں کی پہنچ سے دور رکھیں)
  • Do not use after the expiry date printed on the bottle or outer box
  • Keep the bottle tightly closed after each use to prevent contamination of the solution
  • Discard the bottle 4 weeks after first opening — do not continue to use beyond this period even if liquid remains
  • Do not use if the solution appears cloudy, discoloured, or contains visible particles

Frequently Asked Questions (FAQs)

What is Betnesol-N Ear Drops used for?

Betnesol-N Ear Drops are used to treat bacterial infections of the outer ear canal — such as otitis externa and swimmer's ear — where both infection and significant inflammation are present. Betamethasone rapidly reduces the swelling, redness, itching, and pain caused by the inflammatory response, while Neomycin kills the bacteria responsible for the infection. Together they provide faster and more complete relief than an antibiotic alone.

Why does Betnesol-N contain a steroid as well as an antibiotic?

Most outer ear infections involve two problems occurring together — a bacterial infection and an inflammatory response by the body. The bacterial infection needs an antibiotic to clear it, but the inflammation — which causes much of the pain, swelling, and itching — needs a steroid to calm it down. Neomycin alone treats the bacteria but does not address the inflammation. Betamethasone addresses the inflammation directly and also helps open up a swollen ear canal so that the antibiotic drops can penetrate deeper and work more effectively. The combination treats both problems simultaneously for faster overall recovery.

Is Betnesol-N Ear Drops the same as Betnesol Eye Drops?

Betnesol is a brand that exists in both ear drop and eye drop formulations but they are different products with different formulations and concentrations. Betnesol-N Ear Drops specifically contain Betamethasone and Neomycin designed for use in the ear. Never use ear drops in the eyes or eye drops in the ear unless the product label specifically states it is suitable for both. Always check the label carefully and use only as directed by your doctor.

I am allergic to Gentamicin — can I use Betnesol-N which contains Neomycin?

Both Gentamicin and Neomycin belong to the same family of antibiotics — aminoglycosides — and there is a possibility of cross-sensitivity between them. If you have had an allergic reaction to Gentamicin or any other aminoglycoside antibiotic, you must inform your doctor before using Betnesol-N Ear Drops. Your doctor will assess the nature of your previous reaction and decide whether Neomycin-containing drops are safe for you or whether an alternative is more appropriate.

How long will it take for Betnesol-N to work?

Because Betamethasone is a potent anti-inflammatory steroid, most patients notice a significant reduction in itching, swelling, and redness within the first 1 to 2 days of treatment. Pain relief also typically improves quickly. The Neomycin antibiotic needs the full course to clear the bacterial infection completely — which is why completing all 7 days of treatment is essential even if symptoms feel much better sooner. Stopping early risks bacterial survival and infection recurrence.

Can Betnesol-N Ear Drops cause hearing loss?

When used correctly in a patient with an intact eardrum for the recommended short-term course, the risk of hearing loss from Betnesol-N Ear Drops is very low. The risk of hearing damage from Neomycin arises specifically when the drops are used with a perforated eardrum — allowing Neomycin to reach and damage the sensitive inner ear structures. This is why confirming the eardrum is intact before use is absolutely essential. Report any new hearing changes immediately to your doctor and stop the drops.
